    I had the same pair of extremely light weight and durable frames for at least 5 years previously, but decided to get a new pair because one arm had broken, and although they were able to find a new arm the frame style had been discontinued. (That exemplary service occurred about 1.5 years ago.) When I bought the same lightweight brand in an updated style no one bothered to tell me they were not putting soft nose pads on them. Although the pads fit well, they wound up causing me frontal headaches and facial pain, and ultimately I had to see my eye Dr. to sort out the source of the month long headache. Just two hours prior to that I had requested soft nose pads and was told no by the technician because these $400.00 + frames apparently have an inherent problem, which is that the soft nose pads tend to fall off. My eye Dr. examined me and discussed the chronic headache and its' timing with me and then took me to the glasses department and had them put soft pads on even though the technician again stated they were likely to fall off. Within 48 hours of wearing the soft pads my frontal headache resolved.

    This afternoon one of the pads fell off so I went up to the Mayo Optical department to have it replaced, and asked if I might also be given a spare set of pads just in case I could not get in to the clinic either because of lack of proximity or the fact that they have no after hours or weekend services. She informed me I would have to pay $5.00 per pad and my response was to inquire how they could possibly have sold me a set of frames that had an inherent defect knowingly for the full price. She apologized and stated it was and quot;policyand quot; to charge for extra pads other than those applied directly at the clinic.

    I then went to the check in desk in the Optical department and explained that I was dissatisfied, so was given the opportunity to talk to the RN and then the department supervisor. After speaking with both of them they did graciously provide me with an extra set of nose pads free of charge, but also informed me that I must be more careful and would have to pay for any spares in the future. They advised me that I would need to be very careful while in the process of moving to make sure I didn't bump my glasses. I must note here that I do not bump my glasses, am not a child, and do not appreciate being condescended to and blamed for a situation that clearly has to do with the functional design of the frames sold to me.

    This is not how patients who pay full price, out of pocket, for items that are essential to their ability to function should be treated. It is not how any patient should be treated. I will continue to use the Mayo optical supply department for this set of frames for the two years they are supposed to be guaranteed, but at this point highly doubt that I will order frames through them in the future.

    I did very well, and my post operative course was uneventful. The staff at OakLeaf Surgical Hospital are fantastic, as are the PT practitioners at Northwoods PT. I would highly recommend CVOSM.

    She is not a regular doctor, she is a D.O so she is pretty upbeat and does not always push for testing when I am not sure I should always get every test. The only issue is that it is hard to get in to see the doctors, but I think that is in the way they set the clinic up in general. It takes a few weeks to get in, but you probably can get in sooner if you are sick. It is more like a family clinic and could have a better welcoming environment. When I get to the examining room the nurses see me right away, but it can take a while for the doctor to come in. My doctor seems rushed so I think she has a lot of appointments. They don't take payments at the clinic and I am supposed to send it in, but sometimes the bills overlap.

    A great doctor! Very down to business and effective. A bit cold, but not at all unfriendly. She asked plenty of questions and did not judge or scold. The only bad part was a surprise pap smear, but it was understandable given the irregularities with my menstrual cycle.

    It wasn't long before the burning returned to both feet and I made another appointment with Dr. Pankratz. He then explained that for the past few years, alcohol shots had been known to eradicate some or all of the pain in patients' feet (neuromas), thereby eliminating the need for the surgery that had previously been used to help lessen this foot pain. I then started a series of five shots in both feet over a couple of months; after the last shots, I was still feeling pain, especially in the right foot. The burning that I experienced prevented me from walking for exercise or long shopping trips or being on my feet for any long periods of time.
    So because the pain was still there, he gave me a sixth shot in each foot. I can now be on my feet for a much longer time than I'd been able to prior to the alcohol shots, although not nearly as long as I was able to be before the foot burning started. I am very grateful for the place I'm at now and give huge credit to Dr. Pankratz for his patience and expertise in helping to resolve the problem I had dealt with for so long!

    I felt that my time was not of concern to Dr. Smith.  I saw him for a new patient routine physical and mentioned some symptoms I had been having, which led to the need for several tests being run outside the office.  Once all the results were back, I was scheduled for an appointment to go over the results and waited for AN HOUR for the Dr. to see me.  When he came in, he told me all the results were normal, didn't apologize for running late, and spent about 3 minutes with me.   The office then proceeded to bill my insurance, and I received more than one EOB from my insurance company showing payment more than once for the same service.   A year later, the office called me to notify my that I had missed my yearly routine appointment, which I was not even aware I had made.  Overall disappointed.

    She is very thorough and very caring. She never makes me feel like she is in a hurry. She definitely takes the time to listen to me and answer my questions. I think she is very good and I like her. She is available and punctual. The office waiting room is a little small, though. I don?t like billing system much. The invoices are horrible to read; they are very confusing. I will continue to use her in the future.

    I had spinal fusion operated by Dr. Manz. This visit is annual checkup. Though the drive to see him took two hours each way, but it gives me peace of mind. X-ray of my spine was performed, followed by a doctor consultation. Dr. Manz is very good. He always explains everything clearly.
