    Lisa has been most courteous and understanding. After discussing the insurance problem, I asked about the form that I had sent in August of 2009. She said that she could have Dr. Forman complete it ASAP. When it was returned to me, I had some questions that only Dr. Forman could address and Lisa told me she would ask him to call me back. He promptly returned my call and offered to assist me as much as possible with the documentation that I need. Some of his comments led me to believe that his former office manager had not provided certain information to him. It was important to me that he took the time to further discuss my concerns. My previous review was based on the actions of the former office manager and I continued to recommend Dr. Forman. I am certain that Lisa's efficiency and pleasant manner have been a wonderful enhancement to his practice.

    Dr. Forman's Office Manager was quite rude to me. She initiated a discussion concerning payment/workers' compensation/insurance from across the waiting area. There were several other patients in the room. She reappeared in a few minutes and again from across the room, advised me that the information that I had given her was invalid (her words were less confidential) and she asked for an insurance card. I explained that I didn't think I had it with me. When I later found the card, she made a derogatory comment with an insinuation that I had not been truthful. I felt that her attitude and lack of concern for confidentiality was unprofessional and noted by raised eyebrows and comments from the other waiting patients. I discussed the situation with Dr. Forman and he relayed that she had already advised him of a problem. He was quite kind and explained that this office manager has had similar problems in the past. After my visit, I mailed a disability form to Dr. Forman's office for completion as he had requested. I never received the form back. I called and spoke with the same Office Manager who advised that she would get back to me. I asked her if she needed my telephone number and she stated in a discourteous way that she remembered me and had my telephone number. She did not call back and I subsequently left messages that were not answered. This was over a period of almost 5 months. I once again left a message and a week later she called me back. I asked if the disability form had been received and she responded that payment (a copayment that I was not responsible for) had never been received. (A practitioner who accepts patients with worker's compensation claims is obligated to adhere to the legal guidelines for receipt of payment). When I told her that the payment should be processed soon by the insurance carrier, she noted that she had received an extensive report from the insurer indicating that payment would not be made. She apparently misread the information because I had a copy of the same report which stated that payment would be made. She again was discourteous and pretty much left me with the impression that my form would be held until she received payment.
