    (I used today's date as a service date, but have a relationship with Spectrum that began in 2010 and continues now.)
    Everyone in attendance at this practice is wonderful.
    The facility is wonderfully appointed; in an environment where many of the patients are particularly delicate and require gentle and competent care, all of the staff in attendance - administrative to provider - provide it.
    Mental health care is a tricky field, and one in which finding good providers can be an extraordinarily difficult and exhausting process for potential patients - yet I do not hesitate to recommend that seekers go directly to this center with assurance that they will find kind and appropriate care.

    After finishing the low dose (2mg Buprenorphine 1mg Naltrexone) Suboxone,  I felt no better certainly, but no worse, and the withdrawal from the Kadian was not too terrible.  On the second visit he wrote for the standard 8mg (8mg Bupe-2mg Narcan) Suboxone, enough for one month, even though I was seeing him about once a week.  He's a nice and compassionate man, and kept increasing my daily dose, so that after a month I taking 4 a day.  They're sublingual (under the tongue) tablets, nd take quite some time to dissolve.  The taste is unpleasant to put it kindly. 

    After a month, we decided, and that's one thing I REALLY like about him, he treats me like the medical professional that I am, knowing that I have more research material, books, journals and the like in my library at home than he does in his office.  He NEVER talks down to me, and I respect him all the more for it.   We decided that the presence of the Naltrexone in the Suboxone was interfering with how my brain was picking up receptor sites from the buprenorphine and that Subutex or it's generic would work better.  The insurance company balked at paying for it, so I filed an appeal with them, asking what economic sense does it make to pay for the Brand Name  Suboxone at over $600 a month, and not the generic drug at  less than $200??   I won the appeal for that drug, and was also approved for Bu-Trans, the same drug in a  transdermal patch, approved for sale by the FDA in April 2011. 

    Dr Shimelman immediately phoned in the script for the generic Buprenorphine (It's a Class III, so doctors can do that) .  The next office visit I was doing  so much better he increased my dose to thhe maximum (people plateau at around 40 mg daily) .  He also added the Bu-Trans patch, the 20mcg, highest dose patch, to the regime.   Within a week my pain levels, which never got any better than a 8 out of 10, were now at a FIVE!!!  Also, my functionality increased at least double the first two weeks, in a month it was four fold.  Now I'm happily into my second month, and  haven't felt this good in over five years.   Not to say there haven't been a hiccup or two along the way.  The third patch I opened the package with scissors like always, and it was empty!!  The packaging didn't look to be tampered with in any way, and it's pretty hard to miss a 3"x3" flesh colored "band-aid" type of patch. 

    The drug company, Purdue Pharma LP, gave me such a run-around it was like calling the DMV!  Finally I spoke with someone who said they'd send a postage paid UPS letter envelope to send the packaging to them,  so I did them one better, and sent them the whole month's worth, three patches, four patch envelopes.  (The patches are worn for a week at a time.) Not to get off track too much, but is pertinent, when I called UPS to schedule the pick up, turned out it wasn't totally post-paid.    There was a $9.50 charge for taking the letter envelope out of my hand or mailbox.  I was FURIOUS!!   Purdue insisted they knew nothing about this charge, and thought  it was despicable  (her words) to scam an extra few bucks when they're in my neighborhood all the time.  There are dozens of factories one street over from mine, and  I see the trucks every day!  Many days they turn around in my cul-de-sac, on which I live at the end, if they've taken a wrong turning.  When I called UPS, they suggested I put it in one of the many "drop boxes" in my area.  I told the UPS operator, you expect someone with MS, in a wheelchair, with no car, to try and find a drop box??  He had no response at all.  Yesterday, Sunday the 24th, my next door neighbor very kindly offered to bring to one.  But there ISN'T one in my city.  I can go to the "UPS Store" but only during business hours.  They don't have a drop box, mail slot, nothing.  We went to two post offices, and they had drop boxes for Fed-Ex, but NOT UPS.  I didn't want to take up the man's day or kindness, so I went home, put the whole thing in a manila envelope, and mailed it from my mail-box with my other mail.  I just guessed at the postage,  but I'm sure I put WAY more stamps on it than needed.
    And of course, first thing this morning, UPS shows up expecting  a pick-up, and $9.50 in cash or check for the privilege.  I told him to take a hike, but not that pleasantly.  Purdue Pharma was supposed to call me and let me know if they scheduled a pick-up, they did not, and they can wait another few days.

    As it turns out, according to my pharmacist, they can only get the replacement patch from the Purdue rep.  Theey get them in sealed boxes of four and are not allowed by law to break that seal.   My pharmacist said he sees the Purdue Rep several times a week, and will keep asking about this issue until he gets it resolved.  That's the great thing about independent pharmacies, they know you, and don't forget your problems when it's crunch time.  I had to go without for a whole week, and although I had the sublingual tablets, it was the patch that made all difference. 

    I HIGHLY recommend Dr M.B. Shimelman of New Haven, having worked as an APRN for nearly 23 years, I tend to be negatively critical of doctors as a whole, but this one is a keeper.

    Obviously, the patient base for a practice like this one can be fairly tricky/delicate - they handle every aspect of care with great professionalism and sensitivity, from the initial phone contact/intake, to the privacy and quiet of the multiple soothing patient waiting rooms, right up to the sessions themselves, in which the two providers that I deal with personally - Karen Orzack-Moore, my therapist, and Dr. Daniel Moore, Psychiatrist, for medication management - have proven to be wonderfully knowledgeable and helpful. As anyone reading this report may know, looking for good, consistent, high-quality mental health care providers can be a terrible, taxing process, and there are far more poor examples and resources out there than there are good ones - don't waste any time, and contact this practice right away. They're one of the best.

    Office is small but clean. Appointment was kept on time. Even though procedure was uncomfortable, Dr was kind and had good sense of humor. Explained everything in detail. Good referral of a surgeon.
