    Sharp, Dr. Holly D.
It all started when I went to get a 2nd opinion regarding dental work.  I had asked for my dental x-rays and chart for a specific time period be sent to this other dentist.  This started on October 30 2014.  After 5 attempts (3 phone calls and 2 personal appearances at their office) the information I requested to be released to the 2nd dentist finally arrived in early December - too late to have done any good.  I therefore paid for another set of x-rays.  I was told during this process by the dentist (in a phone call) who worked on me that full mouth x-rays may have been billed to my insurance company but there is no record that they were ever taken in my charts.  He kindly informed me that they would contact Concordia Dental to re-imburse them for this oversight on their part.  Here's is the strange thing.  My bill for those full mouth xrays shows a date of 4/21/14.  By some coincidence when I had asked for copies of my x-rays when still a patient of Sharp family Dentistry, one of the prints I received was dated 4/21/14?  I guess they do exist but maybe they didn't want to release them to me.
At one point, I had shown up in person to take possession of my records after having already signed two release forms.  The office manager told me the Dentist would have to approve my request but he was conveniently in surgery so it would be a two hour wait.  If this were true, that he had to sign for any records I received ( which is what they told me), then why did the very same office manager release a page of my records to me without his signature when I had requested it three months earlier as evidence of my having seen the dentist in order to get reimbursed from a flexible spending account (I had to see the dentist after a dry socket developed following a tooth extraction so I wanted to get re-imbursed for my mileage).
I also informed the office manager of a tooth that was nicked (cut) during the tooth extraction mentioned above so that she could inform the dentist to take a look for himself in the x-rays.  The post-op X-ray (10-23-14) shows clear evidence of the damage.  The pre-op x-ray from 4-21-14 shows no damage.  The office manager never informed the dentist of this even though I asked that she convey this information to him.  The dentist did tell me he would make things right if there was a problem - and I believe him.  But I lost confidence in the office staff.
