  • In Provider name locked. 2014, we received a nebulizer from our doctor's office (retail cost $39.95).  We were told this would be covered by our insurance.  We received an invoice from Provider name locked. dated November 13, 2014 with an initial fee of $23.93 plus a monthly rental fee of $14.71.  The total invoice is $82.77 which is more than double the cost of the device.  I called Provider name locked. , who provides the devices to our doctors office and then bills our insurance company.  I spoke with a billing representative, Provider name locked. , who was very unprofessional and unhelpful.  He insisted that our insurance only covers this item as "rent to own" and stated that they have the right to charge us up to $226 for the "rental" of this device.  The rates they are charging both my insurance company and me are completely unreasonable, as this device retails online for $39.95.  I explained that we had never agreed to a rental and if we had been billed promptly in Provider name locked. , we could have immediately rectified this error and returned the equipment, as we had no intention of renting it.  He blamed it on our insurance company and refused to consider refunding the charges.  I believe it is fair to charge the actual price of the equipment and I would have gladly paid that price, but I consider this business practice of renting the equipment without fully disclosing this information to the customer, is very unethical.  
    We are currently being charged for four months of rental fees for a device we did not agree to rent.  The invoice was sent four months after the fact.  It took three calls before my call was returned, two days later.  The representative was rude and unprofessional.

  • The CAN they have signed would be best for my father.  The assessment was coordinated by a social worked.  She did an assessment of him and the home environment.  Following her assessment they were able to match a very good care giver to work with dad.

  • I give Provider name locked. an A.    It was Citrix that wasn’t great.  Provider name locked. couldn’t do anything without approval of Citrix.  When they were here, they were wonderful.  The first Provider name locked. was very cordial and took good care of him. They were usually on time but sometimes they were delayed.  But they would always call when they were going to be late.
