Most all professional chimney sweeps now utilize the National Fire Protection  211 Standard for chimneys, fireplaces , Vents and solid fuel-burning appliances for chimney inspections, which determines when and whether he has to access the roof.

There are 3 levels of inspection  The most commonly performed are Level 1 and level 2  are listed:

Level 1            Done as Annual Inspection

                      Routine cleaning of the flue

                      Direct replacement of a similar appliance

Level 1 inspection is recommended for a chimney under continued service under the same conditions with the continued use of the same appliance.

IN A Level 1 inspection, your chimney service tech should examine the *READILY ACCESSIBLE portions of the appliance and the chimney connection. HE will be looking for basic Soundness of the chimney structure and flue as well as the basic appliance installations and connections. HE will also Verify that  the chimney is free of obstruction and combustible deposits.

* Readily accessible Means that no special equipment is required to gain such access, ie.Ladder

 

LEVEL 2            Upon sale or transfer of a property

                       After an operating malfunction or an external event is likely to have caused damage to the chimney

                        Addition or removal of one or more connected appliances, or the replacement of an appliance withone of a different type, a different input rting or a different efficiency.

                        Prior to relining or replacement of the flue lining

 

Level 2 insp. are required when any changes are made to the system. Changes can include a change of Fuel Type, changes in shape or material in the flue,Replacement or addition of an appliance of a disimilar type, input rating or efficiency. Building fires, chimney fire or other seismic events as well as weather events or obvious construction defects would be indicators that a level 2 is warranted.

 

Level 2 includes everything in a level 1 PLUS the *Accessible portions of the chimney exterior and interior including attic, crawl spaces and basements. it addresses proper clearances from combustible in accessible locations.There are no speciality tools required to open or access doors, panels, or coverings during a level 2 insp.  The level 2  insp should also include the visual inspection by video scanning equipment or by other means in order to examine the internaql surfaces and joints of all the flue liners inside the chimney structure. No removal or demolition or any permrnantely attached portions of the chimney or building structure or finish shall be required in a level 2 insp.

*ACCESSIBLE means if you can get to it with special equipment do it but you dont have to take apart anything for said access. Use of ladders and cameras just no demolition.

 

LEVEL 3  SEE NFPA 211 STANDARD FOR DETAILS

 

***THE REAL ANSWER to this question lies with the accessibility of the top of the chimney and roof type.  Most professional sweeps want to examine up close the top of the chimney for cracked masonry crowns , which is very common, Flashing issues, top view down flue, and for prefab chimneys to insp the condition of the Chase cover and cap.

Note that the use of a special tool (ladder) to gain access to the chimney is under a level 2 inspection, which can be additional costs since it is a much more thorough inspection.

It is possible to sweep and do a level 1 insp. without accessing the roof. 

 

Also most professional sweeps have equipment that allows us to sweep the chimney with a rotary system that is operated from inside at the fireplace opening and achieve a much better cleaning than pushing brushes down from the top and without getting on the roof. We also video scan the flue from the hearth so the homeowner can be a part of that inspection or "chimney-oscopy". They see the live action of the top of the chimney and conditions that exist inside their system so when they get their report they know they are not some stock pics that could be used to sell repairs but the actual pics of the conditions they viewed in their system. And we do not have to access the roof.

If the chimney access requires multiple ladders or is a "brush with death", there could be extra cost, and the sweep would make an onsite decision regarding the ability to safely reach the top of the chimney or have to use a lesser option of binoculars or rely on experience to determine whats going on up top based on visible symptoms,  if access is too dangerous or not possible.

 

So don't discount a sweeps knowledge or ability on whether he gets on the roof or not. Check him out on referral sites , company web site, credentials, training and experience. He discovered that my chimney didn't have a chimney cap either. Charged me a little more than the original estimate but went a got a custom cap made to fit my chimney's double-wide opening. This is the second time I've had

out to do work on my house. Both times he has been responsive, professional, and performs excellent work for a fair price.
The brick point on my chimney was starting to come loose. $600 to chisel out the old masonry, mix a new batch, and use of a cherry picker that he had to rent to get himself up high enough to fix it.
Same thing with my garage. It's a cinderblock wall and due to a leak in a seam in the gutter above, water was hitting the window sill of the garage and cracking the mortar in the joints in the cinderblocks below it. $250 to chisel out the old and put in new masonry. I had tried this myself several years earlier using a cement mix from Home Depot and a pointing tool, but it looked horrible, and the mixture didn't bond well to the old substrate. Goes to show you, that for nominal money and finding the right guy, the end results are much better and last longer. I'm writing this review in 2015 and the point work still looks great (of course, I replaced the gutter that was causing the water to run down the wall, which definitely helps)
My house is old and some clown decided not to follow best framing practices. They left the entire left-hand side of the main beam of my home unsupported. I had a structural engineer come in to tell me this (hey, I'm no carpenter). But after he pointed it out, yeah, it was obvious the beam needed support.

was down there, in mid-summer, jacking the house up as little as possible and sweating bullets to bang this steel post into place above the footer he poured. He attached it to the main beam with nice thick bolts through the cap plate and the house has been rock solid now on that side ever since. In fact, in 2015, I had my kitchen (on that side) redone and had heavy things like additional sub flooring, tile, granite counters, installed. Everything has remained rock solid.

I purchased a deal here on Angie's List for a $99 chimney inspection and stainless steel chimney cap. I did so because unfortunately, I discovered there were some dead birds stuck inside my chimney because I never had a cap on it. The technicians arrived on a Saturday morning and were very professional. The one technician installed the chimney cap and the other (I believe his name is

came out to do a chimney inspection and sweep. Our fireplaces are in good condition, we maintain them well and keep them clean. There were no issues with the fireplace when we called Winstons for a routine inspection and cleaning. It was the second time we used them in the past 5 years. They did a sweep and said that the chimney was in good shape. I asked the tech about putting in a damper on the roof because I had read that they were more energy efficient. He took out one of the sections of our iron damper to show me how there are several flaps on that kind of damper (our house was built in the 70's) and then put it back. I asked him if the damper was open or closed, he said open and that was it. , He quoted me a price for the newer damper but said that my current dampers worked fine We paid for the cleaning and inspection. The first time we used the fireplace the entire house filled with smoke. The damper was fully open but the entire house filled with smoke, the fire alarms went off for more than 25 minutes everywhere in the house. (on both levels) it was frightening. I called Winstons to ask them to come out and see what happened, and let us know what was wrong. They told me the technician who did the work would call me but he never did. I called back a week later and again was told that the technician who did the work would have to call me back. He never did. From the time they worked on the fireplace to the time we had the smoke, no one but Winstons had touched the fireplace. I was very disappointed that they were so unresponsive. We called another company, they came out and said that the damper had been put back incorrectly. (I did not tell this other company we had had a problem with
