"The second tooth from the back, on the left side of my mouth, had been hurting for a long time. I had a root canal on the tooth behind it about four years ago, and" ever since, the tooth in front of that one (I guess it's called the "first molar") has hurt - not only when I chew on it - but also just when I'm not doing anything in particular. I've seen dentists, endodontists and oral surgeons about it in the past. They thought it was the
tooth behind it (two teeth back!) so they removed that tooth in June 2014 - no relief. Another dentist tried a temporary filling on the tooth - no relief. Dr.
took tons of x-rays, examined my mouth, and tapped my teeth with more interest than the other dentists had done. He pointed out that the tooth that had the root canal (not the first molar in front of it) was actually exhibiting more signs of pain than the tooth I thought was the problem. There may be a part of the root that got missed in the tooth behind it. He could perform another root canal, and he explained that there's a 75%
that will fix it. But if it doesn't fix it, then they will have to extract that tooth. He told me the pro's and con's, and basically said it was my decision. I arrived in his office fully prepared to have a root canal on the first molar, but he explained that it's most likely the molar behind it that's causing the trouble. He said that I don't really use that molar in chewing, so it's okay to have it removed. He said that really, it's what my nature is about gambling... do I want to take the 25%
that it won't work, or just remove the tooth? Dr.
went on to explain that the first molar - the one that I (and my dentist) thought was the one that needed the root canal - indeed had a crack in it. My dentist had put a temporary filling on it, but he said that eventually, it would need a root canal. Dr.
explained that it's the tooth I mostly use when chewing on that side, so it gets a lot of stress. He pointed out that, maybe it would be best to extract the troublesome molar behind it, wait and see how much that side of my mouth still hurts after I heal up, and then decide what to do. He pointed out that I could just get a crown on the first molar, and that might last me several years, postponing the need for a root canal. Again, he pointed out that it's my decision, but he wanted me to understand what was going on, and have all the facts. For a guy who makes his money doing root canals, I was really surprised that he basically talked me out of it. I'm going to see an oral surgeon to have the back molar extracted, wait for the area to heal, and see if that first molar still hurts. I felt like he really understood dental work, and provided me with options. I mean, what if he had just done the root canal on the first molar, I went thought all that pain and expense, and that side of my mouth STILL hurt because the back molar hadn't been addressed? Dr.
saved me some time and anguish. I really trust him, and I would go out of my way to see him again, if I ever do need a root canal.

"Although not super warm and fuzzy, Dr. Roumani seems highly competent and professional, which is good enough for me. Desk staff is fine enough, but dental assistant" staff really shines IMO.  I am familiar witht he ClearChoice Dental Implant Centers across the country, in particluar, the one in Overland Park, KS.  They are a "one stop" implant center.  The doctors there are at the top of their field, and an advantage is that you have all of the specialists that will be working with you in the same place.  That would have to make communication easier between doctors.  They usually offer seminars for patients and I think, offer free consultations, which is always worthwhile.

  If you chose to go the other way, some Prosthodontists will place and restore the implants themselves. In other cases, an Oral Surgeon or Periodontist will place the implant and a Prosthodontist or General Dentist restores it.   Fees can vary quite a bit from office to office and with the different options for restorative materials.  Some offer free consultations, for others, they charge a fee that might be applied to any future treatment in some cases.

The first step is to schedule a few consultations with different providers, which it sounds like you've already done.

The most important factors when deciding which practitioner to work with:
:
.1.  You should feel comfortable asking your doctor any questions so that you have a good understanding of the procedure itself, healing time and long-terem outcome, and possible complications.  Write down a list of questions and take them with you to the consultation.

2.  Ask about the experience of the practitioner. How many similiar procedures have they completed?  Have they had any major complications with any cases?  Ask to see photos if possible.

3..Figure out if it works out better for your schedule, to go to a "one stop" provider or to separate providers for the placement and the restoration of the implants.

4. Follow your instincts.  If you feel rushed, didn't get direct answers to your questions, didn't communicate well or had any other negative feelings, this probably isn't the doctor for you.

5. Consider insurance coverage and financing options.  If your dental plan covers implants and it is important to you to use a preferred provider, you will want to check your list.  Many times, insurance does not cover implants, or at least not all of the steps.  You may want to inquire about payment and financing options.

As a  Dentist, I am sorry to hear about your denture nightmare and I know how frustrating that can be.  I am surprised that your doctor wasn't willing to work with you on some type of solution.  A few options would be either remaking the dentures once more,  or offering to return some of the money and have you return the dentures.   Unfortunately, the doctor has put in a lot of their time and lab expenses, but the patient also  has time and money invested.  You might try sending a letter re-explaining the problems you hand and offering an acceptible and fair solution.  Be sure to keep a copy of it and any correspondence you receive from the Dental Office.  In the event that you don't get anywhere, you have documentation of the problem. 

At any point in the process, did they have you sign something saying you were happy with the denture set up?  Some practitioners will document the patient's acceptance at the try-in appointment, before the denture is processed.  At that time, changes in tooth arrangement, size and color can be made with minimal lab expense. 

If the letter doesn't get you anywhere, you might try going to the governing body of the dental profession in your area.  I am a dentist in Blue Springs, Missouri, USA.  Here, we have the Missouri Dental Board  (or the Dental Division of the Missouri Department of Professional Registration.)  This is the entity that licenses us to perform our professional duties in our state.  They often have a process to file complaints against a provider if you were unable to resolve the problem yourself.   After your complaint is received, they will contact the doctor to get their side of the story, then try to mediate a solution between the two parties.  

As far as a new set of dentures, you might seek out the services of a Prosthodontist (specializes in dentures and removable appliances - extra training and certification beyond general dentistry) or go to a local dental school.  The students are supervised and checked off at each step by professionals.  Most dental schools also treat patients through their faculty practices, and you can often get the services of a specialist at a lower price.

for 4-5 years now. He has actually completely restored my smile. It's because of him that I can smile today without gaps and yellow teeth showing. Today he removed a tooth. The procedure was not painful at all. The doctor frequently makes sure that you are not in pain and if you need a break, he gives you a break. I have always been able to tell that he and his staff are attuned to my needs as a patient.
I can tell you the reason I chose him years ago to restore my teeth. After talking to a few Dentists, and having bad past experiences with some, he made me feel comfortable. And he communicates with you! He wrote a plan sitting next to me and told me how hew was going to address each problem tooth I had. As things turned out, I needed several root canals and lots of fillings. i have had all that work done at his office. Some dentists send out root canals and other procedures to Endodontists etc.. Dr.

is qualified and does those procedures all at his office. This eliminates finding a problem, getting a referral, calling a strange doctors office. Then you sit on their waiting list for a initial exam, and 6 weeks after the problem was found something gets done. He once did a emergency root canal within 24 hours of seeing it was needed for me.
I actually have a check up appointment tomorrow at his office. I am sure I can expect the same quality service and dental work when I go. I am grateful he and his staff have restored my smile.
