ProActive accepts PPO, HMO, Personal Injury, and Workers Compensation Insurances. A copy of your insurance card and driver’s license will be required to verify your coverage prior to treatment.
15 % discounts are available to cash patients with conditions requiring multiple treatments per week, college students, elderly above 70 years of age.
