Our members receive a memberhsip card with their name and a code number printed on it, also our office number. A call to our office with their name and code number is all that is required. And for providers, if a medical provider wishes to join our professional medical staff, they may call and set an appointment at 949-463-2678, and a representative will call on you to review your requirements.
