"They were absolutely outstanding. The medical staff communicated with me, even though they knew I was the sister-in-law. The therapists communicated with me. Everyone treated my sister-in-law" and me with dignity and respect. The food was excellent. The accommodations were great. It was a lovely place, and I understand why there is almost a waiting list to go there. Their goal is to rehabilitate people and move them out; not keep them there for 20 days. My sister-in-law was there for less than two weeks, and she so vastly improved that it is absolutely remarkable.

& Rehabilitation Hospital" is not the type of medical facility that one goes to voluntarily or intentionally!
In the vast majority of cases patients at this facility who are on Medicare and thus are perhaps enrolled in a local Medicare HMO (e.g., Senior Dimensions before takeover by United Healthcare) like my mother have no choice but to go to this facility since this is the only option that their health insurance plan covers.
Visits by physicians are sporadic and brief, many if not most physicians are trained overseas and are still on the road to learning proper spoken and written English (thus occurrence of nonsensical phrases such as “deFooter” in discharge letters – pray tell what might a “deFooter” be?).
Written treatment plans from the initial acute care hospital are not followed/ignored, most likely because the discharge documentation was never read by the attending foreign-born physician responsible for a patient’s care at the rehabilitation facility.
Apart from patient medications initially not being correctly provided as specified b y the referring hospital (Valley Hospital), nursing staff apparently does not know that patients need a minimum of approximately 1.5 – 2.0 liters per day of water on an ongong basis to survive. Proper oral care is not provided to patients (especially the demented) who can no longer provide such care on their own. Thus, the risk of aspiration (food & drink) and of developing aspiration pneumonia are increased.
Administrators (a Mr.

at the time) do not respond in any way, shape, manner or form to numerous certified letters sent “Return Receipt/Requested” concerning important patient-related healthcare issues at the facility.
Bearers of the ultimate in bad news (such as you are going to die!) do not even have the professionalism or common sense to first properly introduce themselves when talking to worried and stressed-out family members, and oh so much more than I can relate here.
When it became apparent that my mother - whose swallowing had been evaluated and cleared - was not eating and drinking enough to survive over the intermediate to long-term, I considered whether or not something might be preventing her from doing so other than the fact that her time had come and she truly couldn’t eat and drink enough anymore (part of the natural dying process as the body shuts down).
It turned out that she had a urinary tract infection (UTI) and infections are known to negatively impact cognitive (mental) functioning in dementia patients – make them more confused, somnolent.
The UTI was only diagnosed at my suggestion/insistence; the staff would not have been motivated to do this on their own. Apparently,

as a urine dipstick test available to quickly and efficiently screen for potentially bothersome, patient-impairing UTIs in catheterized patients.
This was not supposed to happen at

Rehab since the treatment plan accompanying her transfer documentation from Valley Hospital clearly stated that she was to remain free of infection (indwelling catheter
Pittsburgh Physical Rehabilitation Services Provider Name Locked
) and that pertinent laboratory values (ESR, leukocyte count, CRP) were to be regularly monitored to ensure that this was the case. None of this was apparently done according to the medical records.
Since my mother had an indwelling urinary catheter (

catheter) first placed when she was admitted to the Valley Hospital ICU on 02/12/1007 and such a catheter remained in place until she died 44 days later, medical staff at all of the healthcare facilities where she was a patient should have been aware that she would inevitably develop a (

) catheter-associated urinary tract infection (no longer than 3-4 weeks at the utmost). This is undoubtedly what happened to her, as far as the UTI that she developed was concerned.
When my poor mother was being transitioned from

Rehab to hospice care (was supposed to be home hospice with an initial stay at the
Pittsburgh Physical Rehabilitation Services Provider Name Locked
Hospice IPU to treat her UTI, but she actually never got to go home again) on 03/23/2007, the expectation was that she would be receiving all of her needed medications while being given intravenous fluids and having her UTI treated with antibiotics (Levaquin i.v.).
If the UTI was indeed the culprit in preventing her from eating and drinking enough to survive she would resume doing so once it had been successfully treated. Otherwise, she was going to die a natural death due to inadequate hydration/nutrition within about 1-2 weeks.
The plan sounded very reasonable and feasible, but alas

& Rehabilitation Hospital and its incompetent staff threw a monkey wrench into our well-thought-out plan.
I didn’t find out about this until after my mother had passed away much more suddenly than expected in hospice care (only 5 days after admission), apparently suffering from end-stage respiratory failure and congestive heart failure.
After obtaining copies of all of her medical records from

Rehab at a cost of some $600.00, it was very clear that she was also suffering from pneumonia (discharge chest x-

were available at 10:30 a.m. the very next day, Friday – her day of transfer to hospice care.
She did not leave

Hospice until after 1:00 p.m. on Friday, 03/23/2007. Certainly there was enough time for someone to find out the results of her chest x-

before she left curative care for hospice care – if that is what they had wanted to do.
Hasn’t anyone ever heard of a telephone call or to specifically request that the x-

results be immediately faxed as soon as they are available, because the patient is being discharged the very next day? It makes absolutely no sense to order a test – such as a chest x-
Pittsburgh Physical Rehabilitation Services Provider Name Locked
– which then comes back showing serious pathology, never look at it and never report the findings to the affected/interested parties.
If a test is of absolutely no consequence, why order it in the first place? If you order a test and it comes back showing something important, you better “get off your a** pronto” and provide appropriate follow-up in a timely manner!. No follow-up was ever provided to the

Hospice (by phone or otherwise)!
I strongly believe that we and the hospice were intentionally not told that my mother had pneumonia, because it was known that this would very likely delay her transition to hospice care. We might have been willing to transfer her to hospice care with a urinary tract infection (

Rehab didn’t want to keep her with just a mere UTI!), but pneumonia very likely would have meant that we would have wanted to pursue some additional treatment (antibiotics) in curative care first, before agreeing to hospice care.
With a capitated patient like my mother who had been at

Rehab for 23 days (and thus was approaching the maximum covered day count in rehab under capitation), there was certainly a financial incentive of the part of

She’ll be dead in hospice care before they know it and no one will be any the wiser about what really happened. Well they didn’t count on me requesting copies of her medical records on the very day that she died in hospice care – THE BASTARDS!
This story has had a long aftermath dragging on for several years after the events in 2007: correspondence with the CEO of Fundamental Long Term Care (FLTC) - the parent company of
Pittsburgh Physical Rehabilitation Services Provider Name Locked
& Rehabilitation Hospital, an agreement (copy available) to compensate us the princely sum of $1,000.00 for expenses (which we declined not only because we would have had to keep our mouth shut about what happened) related to medical records and the need to obtain an amended death certificate from the

County Coroner’s Office (ultimately only possible because an accidental death – fall), since

Ultimately, this ended up as a complaint with CMS (Centers for Medicare & Medicaid Services). Supposedly several people at

Rehab were subsequently terminated, but I cannot confirm this.

I went to therapy twice a week (ultrasound, stripping, exercises to strengthen and increase range of motion, electrical stimulation and cold) and did therapy exercises three times a day at home. Within two weeks my foot was getting stronger and my pain level began dropping. I decided to try going back to work on restricted hours. I was able to tolerate a regular shoe for 6 hours a day which I had been unable to do before I began therapy. After two weeks back at work we increased the level of therapy and I had a setback and my pain level began increasing. We dropped the therapy back down and I began to bounce back. I went back off work to just concentrate on the therapy, but after another week and a half, I realized I was no longer continuing to improve. I talked to

and we decided to stop the therapy and recommend to the doctor that I should have the surgery to remove the bones. Even though the therapy didn't fully work, my foot is stronger and I have less pain than when I started therapy so that should help me recover from surgery better. I am looking forward to working with the people at

again for rehab after the surgery.

was like a miracle! He went from not being able to tell us his full name and address to now being his old self again. The entire staff that cared for him was unbelievable. He received Occupational Therapy, Physical Therapy, Speech Therapy, Recreational Therapy, as well as daily sessions with a Psychologist and Physiatrist (MD specializing in physical rehabilitation). There was also a Social Worker who was assigned to his treatment team. His entire treatment team was in constant communication with me.
I couldn't believe the quality of care he received and the dedication of each Therapist who cared for him. After about 4 weeks at

he was discharged and continued with outpatient therapy at Northern Pines Medical Center in

I have had sever stenosis for the past two years. I could not stand up or walk. Once I met with Dr. Meleger, I was diagnosed quickly and was given the choice of either injections to the spine or surgery. Of course. I chose the injections. I am now going in every three to four months to have these injections once the pain re-occurs.

Consequently, we believe that this matter was handled in an extremely poor manner -- very poor customer relations. Thus, we would not recommend this facility to anyone.
