"Mixed bag experience. Food service was good with a variety of healthy choices, room service was good as well. Activities were also good." The nursing itself could use some improvements. I some times had to wait up to an hour and a half for pain meds. I would only use them again if the nursing staff improved.

"They do absolutely nothing we pay top dollar for her to get assistance and beg them to keep her hands and fingernails cleaned and give her a daily bath. This apparently" is only happening once a week if that and only if we call and complain that it's not getting done. These people are incompetent and talking to them it's like they say they will do it but it never gets done, so frustrating. My mother is constantly getting staph infections because she is not being kept clean.

The first evening he was there he wandered out into the parking lot, despite having a device on him that should have prevented that. The second day he was in their care he fell and burst his spleen. He had to be hospitalized and died ten days later.

This place needs to be shut down because they do not give attention to their patience.

My Mom fell and broke her hip and wrist several months ago. I first placed her in another facility, which caused further physical damage to her, and I had to move her quickly. Benedictine was recommended highly, and they had a spot for Mom. We have not regretted the change at all - they have been responsive, caring, competent, and really try to do what ...More they can to minimize the obvious sadness and stress of being hurt and away from home. The building is old and somewhat rundown - who cares? The main requirements - restorative therapy, compassion and encouragement, interest in individual patients and their well-being, any physical surroundings. I've been nothing but grateful to have found Benedictine, and only wish I lived a little closer!

I tried to get referrals/reviews/any information on skilled nursing/rehab facilities nearby. This facility is a new building/location, but is actually a relocation/update of an older "Avamere" nursing home nearby.(Avamere is a large corporation which owns many Assisted Living and Nursing/Rehab centers). I had heard really negative things about ...More the "old" facility, but was told by a number of sources that they hadn't heard anything negative since the new facility was built, and thought things were good now... One of the resources I spoke with, "A Place for Mom", gave a thumbs up to this Avamere nursing home. From the day Mom (95 years old) was transferred here, things were bad. She was in a lot of pain, was weak & confused, and had pre-existing medical conditions that required care and vigilance. She has essentially no circulation in her legs/feet, and any damage can quickly become a major injury. Despite my warnings, and even when I pointed out damage that was starting, her feet were injured by pressure sores on her heels (which became necrotic/black) because they were not "floated" off the surface of the bed, but left in one position. Pressure stockings (very tight/to knees) were applied in a manner that literally tore up the tops of her feet, resulting in severe deep wounds to both feet. Response to these injuries was lackluster, and even after instructions by the Nursing Manager were given (in writing) leave her feet bare and float her heels, the next day they were again flat on the bed with the stockings on her over the wounds... Mom is legally blind (total central vision loss), and had her right hand/arm in a cast, but was given minimal, if any, help with meals. Residents were dressed in "street clothes" and wheeled to the main "beautiful" dining room (even when this was a painful process) for meals, where CNA's acted as waiters/waitresses... I accompanied her to dinner one night, and she was brought a large white ceramic dinner plate, with a 3 . piece of skinless dry chicken breast, heavily coated with sprinkled "herbs" sitting alone in the center - and nothing else on the plate. No veggies, no potatoes/gravy, nothing else! Oh, she had a glass of water... When I protested, I was told "that is what she ordered..." (Some of the staff didn't even know/realize that she is blind...) She was in that "home" for slightly under a week, and by that time was so damaged - in worse shape than when she arrived - that I had her transferred to another facility - a GOOD one! She has been at the new facility - the Benedictine Center in Mt. Angel, OR - for about 6 weeks now. The tops of her feet, which have been receiving daily nursing care, are slowly healing, although still very damaged. Her heels are still severely damaged and potentially may never heal. She cannot wear shoes at all, and not even most slippers, and has prescription sheepskin-lined "boots" to cradle and protect her heels. There were other issues with this place. It has been a horrible experience, and I can only hope other people can avoid the pain and damage Mom has been subjected to. (I have photographs to document the damage to her feet, and even a photo of the pathetic "broiled chicken breast" dinner she was served). This facility brags about their high review-ratings, but careful attention to their Facebook reviews shows the majority of their "5 star" ratings are from current or former employees, who simply add the 5 stars without any actual information. I will probably feel guilty forever, but there just wasn't legitimate information available to know that I was essentially sentencing her to H***..

My Mom was a resident here from Sept. 2012 - . 2013. This is a Medicare/Medicaid regulated facility. Upon admittance, we were asked to be on call 24/7. My sister and I agreed to this. After 4 .,I was told by the Nurse Supervisor they were transferring my Mom to a Psychiatric hosp. about 1 1/2 hrs. from ...More my home. This was a shock! No prior concerns or mtgs were given to me or to my sister about this move. We requested a mtg.,which did occur. After another mtg., we requested help from the Ombudsman"s Office, since we feared this would be an unsafe and unwarranted transfer. We were very concerned re: Mom's behaviors changing, as we observed, previously, due to urinary tract infections(UTI'S). She had almost died due to a urinary tract infection that resulted in Sepsis at another facility. She had been hospitalized for 12 days. After many denied attempts to have Mom seen by urologist, we did confer with state inspectors, who were there at the time, and did relay our concerns. The Ombudsman never set a meeting time to address our concerns and after relating them to the state inspectors I was sent a letter by regular mail, that I was jeopordizing Mom's welfare. We had to be supervised in her room, when visiting. This was done by, at times, the lobby attendants, and a nurse's who had fallen asleep,when I had to leave the room. I tried to contact county freeholders, since this is a county facility, but to no avail. We were later told that we were disruptive and visiting was limited to only outside of Mom's room, with staff present. We were then told that was filing for guardianship of our Mom. We also filed for guardianship. I did have financial POA. After Mom's appt. with an Infectious Disease Dr., instead of a urologist, we were told that we were further disruptive, and visitation was limited to 11/2 hrs. each., in front of the nurses' station. We were previously with Mom for at least 6 hrs. per day. This occurred after the Infectious Disease Dr. appt. in which the court - appointed temp. guardian had to assist me with Mom's toileting needs. had sent Mom to appt. unaccompanied (I was not allowed to go with her as I had been doing on past appts.). She was sent without any hydration and without any of her toileting needs, due to some incontinence issues. This was in and it was at least 85 degrees. My sister and I had brought juice and thickener and all of necessary products for her toileting needs. We were not permitted in the room during Mom's examination by Infectious Disease dr..That evening at about 6:00 PM, I was sent an email re: our further disruptive behavior. We were not even at the facility that day. We were with Mom at appt. . Previously,we sat outside of Mom's room when she repeatedly asked us to come in and be with her. We were told not to enter her room and not to sit with her at meals. Visitation with Mom was then limited to 11/2 hrs. per day each. We had previously been banned from her room. We would sit outside of room while she asked us to come in at bedtime.We used to sit and pray with her and she would fall asleep within mins. and this was no longer possible. Mom kept asking us to come into the room and we couldn't. Guardianship was finally awarded to me after 8 . The judge never even met with us. This is a county facility and it seems that Medicare/Medicaid Guidelines aren't adhered to here. Visiting with family is to be private and at is 8:00 AM to 8:00PM. The judge said that she would not adjust visitation hrs., even though I was awarded guardianship. My Mom would call out for me most of the day. Other residents reported this to me. My Mother was denied the right to see the neurologist regarding her Parkinsonism and tremors.This also goes against Medicare/Medicaid facility regulations.
The tremors became so severe that she could not hold her teacup.Her voice became very hoarse and she complained of throat and chest pains. We did relay these to nurse at facility.
Mom had many unexplained bruises. I requested Mom's medical records and this was denied until after 1 mon., and then she was finally home with me. Mom suffered with internal bleeding and was repeatedly hospitalized. She died after 3 .,in March, 2014, since her veins could no longer be utilized for a transfusion. There are many other documented issues regarding this facility. My Mother suffered so much by being denied her rights to be with her family and to see an outside physician. I only hope to spare other families this awful experience. Thank you

My father in law left after 5 days. He was readmitted to the hospital with dehydration and renal failure. He did not receive a bath in the 5 days He was there. I picked up trash from the floor of his room on each visit. CNAs slow to respond. Several times I would go to find them and they would be sitting with feet up texting on personal cell phones. ...More Learn from my mistake. So not use this facility.

Since my family had a short time to visit skilled nursing facilities before my father left the hospital, gave us a quick appointment so we could tour the facility. Questions were asked and answered, as well as information about the facility explained. When we arrived on a Saturday, the staff was very friendly and gave my dad ...More a thorough examine and explained everything in his room.. (He had a a private room...very modern and well lit). Since my father had many health issues, the aides were very kind, patient and helpful. An aides station is set up at the end of each hallway. They are very good at responding to help buzzers. (Sometimes he needed to wait while aides assisted others). The bathrooms were kept clean by housekeeping and trash emptied daily. One particular , , was very good to my father. My father just thought the world of him. He showered, dressed him, and wheeled him to lunch/dinner. He always had a kind reassuring word for my dad.
Meals were served in the dining area. My father met and became friends with several of the men. The nurses were very efficient at distributing medication too. They always stopped what they were doing to answer any questions I had. I visited everyday and each time I found a pleasant environment. I also liked that I could visit at any time of the day. Laundry rooms were available free of charge to wash clothes, however, I did my father's laundry at home.
The PT and OT staff were very friendly and answered any questions I had. They also were efficient at making sure service was provided to my father everyday. If they missed a weekday, they would make it up on the weekend. My dad also liked the therapy dogs that roamed the area. When it came time for my father to leave, a staff meeting was arranged where my family and father met with a social worker, RN, PT and OT. They explained to us what they had done and the services my dad would receive at home. All in all my family was very pleased with the services and environment provided by .
